Job Description:

Hourly Pay Rate Range of $20.00-$25.00

Provides occupational therapy services under the supervision of designated licensed and registered occupational therapist. Responsible for collaboration with QI Coordinators, QI LPN, and QI RNs during review of programs. Participates in staff and person served support and education. Other duties as required.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES

---Review of PT, OT, Speech protocols and Dietary Order Letters.

---Reviews meeting minutes as needed for any applicable committee meetings (i.e.: falls, wound, etc.) to be aware of person specific / site specific needs.

---As a member of the Interdisciplinary Treatment Team, participates in professional meetings as assigned, integrating services with other providers.

---Involvement in mentoring new employees and COTA students.

---Makes recommendations for the purchase of specialized equipment, materials, and wheelchair modifications.

---Counsel person served and their family members/care givers (if applicable) regarding the person's Rehabilitative Health Services including staff/family education.

---Recommends revision of written plans/ staff actions/ protocols to agency rehab clinical staff.

---Provides screening for potential Rehabilitative Health skilled services.

---Responsible for completing a summary (implementation plan) at the conclusion of a review/support with findings and action items.

---Ensures completion of all agency mandated trainings, to include CPR & SCIPR certification.

---Maintain productivity expectations.

---Provide an exit conference at the conclusion of the review.

---Maintain 90% compliance on Chart Audits and follows required documentation guidelines/expectations set forth by the department, insurances and DOH/OPWDD.

---Other duties as assigned.

---Demonstrates flexibility by understanding different views. Adapts approach as situations change and accepts changes in the organization and job description requirements.

---Supports clinic/agency by accepting additional tasks outside of job description such as special projects, public education.

---Plans time off in advance and manages PTO according to department guidelines.

MINIMUM QUALIFICATIONS

---Graduate of an accredited program for Occupational Therapist Assistants.

---Completion and maintenance of current National Certification by the NBCOT (National Board for Certification in Occupational Therapy). New graduates must have this completed within 1 year of employment.

---Must be currently registered to practice in New York State, in compliance with the Department of Education.

---Meet requirements for continuing education in accordance with the NBCOT and New York State Department of Education.

---Lifting requirement of 35 lbs.

---Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention Core Training certification.
