Our welcoming campus includes a variety of senior apartments, assisted living, memory care, skilled nursing care, and state-of-the-art rehabilitation. - -MDS Coordinator--------------------- - -JOB SUMMARY:Conduct and coordinate the development and completion of the resident assessment process in accordance with the requirements of the Federal and state regulations as well as Company policy and procedures.ESSENTIAL DUTIES:Oversee and coordinate the development and completion of the resident assessment (MDS) in accordance with current Federal and state rules, regulations, and guidelines that govern the resident assessment, including the implementation of CAA's.Assist in the development of a comprehensive resident assessment and care plan for each resident as chairperson of the facility Interdisciplinary Care Plan Team.Communicate with HMO's as needed to provide updates.Assist in giving 48 hour notice.Respond to requests for denials or appeals from Medicare, HMO's, etc..Disseminate any new or updated materials involving the RAI process.Facilitate the involvement of appropriate health professionals needed to improve or maintain the resident's functional abilities - at the highest predictable level.Create an opportunity for family participation in the care planning process.Ensure the face validity of all Minimum Data Sets before electronic submission.Participate in and oversee the timely electronic submission of all MDS.Review the validation report and ensure that appropriate action is taken.Serve on, participate in, and attend various committees of the facility (e.g. Quality Assessment and Assurance) as required, and as appointed by the Director of Nursing Services or Administrator.Conduct monthly review of random sample of a minimum of 2% of the MDS to ensure accuracy and internal consistency with the medical record.Work collaboratively with the DON to ensure that all new nursing associates are knowledgeable of the RAI process and receive appropriate training.Communicate with the Business Office Manager and Administrator on a regular basis regarding the case mix scores and how they impact reimbursement.Comply with all policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts, and personal protective equipment.Understand, comply with and promote all rules and regulations residents' rights, promote positive relationships with residents, visitors, and regulators.Other duties as assigned. -POSITION SPECIFIC COMPETENCIES:Demonstrates clinical assessment skills at the level necessary.Ability to make independent decisions.Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.Effective interpersonal skills.Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, and policies and procedures that are necessary for providing quality care.Capable of researching new methods and principles to incorporate them into existing nursing practices.Proficiency in Windows- Word, Excel, and Power Point.Proficiency in written and oral communication skills.Proficiency in decision making and analytical skills.Must be able to render safe and efficient care based on Rosa Coplon policies and procedures and NYS Department of Health guidelines. -WORKING CONDITIONS:Must be empathetic and sensitive towards and able to work with the geriatric population.Chance of exposure to human bodily fluids. -PERSONAL OR PHYSICAL REQUIREMENTS:The physical demands described here are representative of those that must be met by an Associate to successfully perform the essential functions of this job. Ability to lift, pull and push a minimum of 30 lbs.Must be able to communicate with residents, supervisors and coworkers effectively.Ability to push resident's wheelchairs.Must be able to visually assess residents and render appropriate care. -EDUCATION:Bachelor's Degree in Nursing (BSN) (preferred).OR Associates Degree in Nursing (ASN) (required).AND Current and Unencumbered License to practice nursing in NYS (required). -EXPERIENCE:
Three (3) Years of Nursing Experience (required).Two (2) Years working with the frail or elderly (preferred).Two (2) Years of MDS experience (preferred).Powered by JazzHR
