DescriptionUnder supervision of the Director of Patient Services,
and in accordance with agency standards and policies, gives
treatment, nursing care and health instruction to patients in their
home. ESSENTIAL FUNCTIONS /RESPONSIBILITIES: Under Supervision,
adapts hospital procedures to home situation to give skilled care
to patients requiring part-time professional service, and initiated
nursing measures to prevent complications or to minimize the
disabling effects of disease or injury. Observes, evaluates, and
reports to physician patient's physical and emotional condition,
reaction to drugs and treatments. Interprets to physician social
and physical factors in the environment that affect patient care.
Interprets to the patient and family/staff complications of the
diagnosis and the nature of the treatment consistent with the
directives of the physician, and teaches and supervises family
members or others giving care. Plans for coordinated nursing
service for individuals and families/staff under his/her care.
Maintains necessary records for analysis and planning of service,
and for the establishment of priorities for care. Performs
diagnostic tests as authorized and interprets findings of tests to
individuals or families. Obtains laboratory specimens when
indicated. Gives preventive immunization or treatment under medical
direction. Provides family/staff with information, support, and
encouragement which may help them adopt attitudes and practices
that promote health and reduce anxiety, tension, and fatigue.
Consults about patients health problems with other professional
workers, such as nutritionists, social workers, physical therapist,
occupational therapist, speech, psychologists, and psychiatrists.
Helps the family/staff to accept appropriate medical, hospital,
nursing home, welfare, or other care necessary. Interprets extent
and limitations of services available. Arranges referral and
communicates pertinent information to the agency. Participates in
Team Conferences. Maintains confidentiality of patient and agency
information. Is responsible for completion of required paperwork in
a timely manner. Maintains productivity standards. Responsible for
insuring that all confidential and potentially sensitive
information is processed, maintained, and utilized according to
procedures in a strict confidential manner. Other duties as
assigned. Minimum RequirementsMust possess a current license and be
registered to practice in NYS as professional nurse. Must possess a
valid NYS Driver's license. CPR certification. Must possess an
operating vehicle. Home Health care experience preferred; 1 year
Medical/Surgical experience or equivalent required.
