Job Description Locums Primary Care Provider (PACT Model) Outpatient Adult Primary Care | Veterans Affairs Canandaigua, New York Rate: $130/hour Contract Length: 13 weeks Schedule: Monday–Friday | 8:00 AM – 4:30 PM Setting: 100% Outpatient (No weekends, No federal holidays, No call) Position Overview We are seeking a Board-Certified or Board-Eligible Primary Care provider (Internal Medicine or Family Medicine) for a 13 week locums assignment providing outpatient care to Veterans in Canandaigua, NY. This role follows the VA’s Patient Aligned Care Team (PACT) model, delivering coordinated, team-based care in a structured outpatient environment. Scope of Work Adult outpatient primary care 12–15 patients per day Panel size approximately 1,200–1,400 patients Continuity of care including prevention, diagnosis, treatment, referral, and follow-up Care delivered within a collaborative team model including: 1 Provider 1 Registered Nurse 1 Licensed Practical Nurse 1 Medical Support Assistant Clinical Responsibilities Routine exams and follow-up visits Chronic disease management (HTN, DM, COPD, CHF, hyperlipidemia, etc.) Preventive screenings and health maintenance Women’s health services including preventive screenings and gynecologic evaluations Psychosocial assessments and coordinated care planning Documentation within the VA Computerized Patient Record System (CPRS) Participation in interdisciplinary team meetings Qualifications Board Certified or Board Eligible in Internal Medicine or Family Medicine Active, unrestricted U.S. medical license (any state accepted) DEA registration BLS & ACLS certification Eligible for VA credentialing and background clearance Contract Details 13 week locums assignment Travel and lodging provided Malpractice coverage included No call responsibilities If you are interested in a structured outpatient role with predictable weekday hours and strong clinical support, we welcome the opportunity to connect.
