Job Description Job Description Description: Summary : Under the
supervision of the Community Based Program Coordinator, the Job
Coach is responsible for providing service coordination to
consumers who are participating in the Supported Employment
program. The Job Coach is also responsible to organize, implement
and train the participants with workshops to provide job and living
skills. Essential Functions: Reasonable Accommodations Statement To

Employment Programs Coordinator. · Creates connections and
partnerships in the community to secure job opportunities for our
clients. · Establishes and maintains working relationships with
employers, industry organizations, and public agencies to source
job openings and secure job opportunities. · Responsible for
creating job opportunities for clients/ individuals by researching,
identifying, and soliciting commitments from possible sources of
employment. · Creates and teaches job development training
curriculum. · Provides training to individuals in all aspects of
employment. · Provides assessment and ongoing service to assigned
caseload · Maintains accurate records and progress notes for each
client. · Assists in the development and implementation of an
individual’s vocational goals. · Assists the program coordinator in
job development and recruitment of program participants. · Attends
formal and informal training seminars relating to the Supported
Employment Programs for Developmentally disabled adults. · Provides
advocacy according to the participants' needs. · Organizes and
implements workshops on job readiness as well as independent living
skills. · All other duties as assigned by the supervisor.
Authority: The employee exercises considerable initiative and
independence in scheduling meetings with program participants,
referring agencies, and attending training seminars. Maintains the
competitive framework of this program as established by policies
and procedures of the agency and OPWDD. Requirements: Position

clearly and concisely. Any other job assignments required by the
agency/ supervisor. Skills and Abilities: · High school diploma. ·
Experience working with community human service providers and
working with the Developmentally Disabled population. · Knowledge
of community human service providers. · Bilingual in
English/Spanish. · Valid NYS driver’s license and reliable
transportation. · A combination of training and experience other
than the specified, if judged to be adequate for the job, may be
accepted by the President and CEO. Physical Demands: The position
does require occasional standing, squatting, and lifting of up to
approximately 10 lbs.
