LOCATION: Monroe County DEPARTMENT: ElderONE SCHEDULE:
Monday-Friday, Day Shift SUMMARY RN- Home Care promotes and
maintains health of individuals in the community through teaching,
counseling and appropriate preventive and rehabilitative services.
Responsible for assessment of health needs and contributing to the
plan for care for individuals and families. ElderONE An affiliate
of Rochester Regional Health, a Program of All-Inclusive Care for
the Elderly (PACE). Servicing Monroe, Ontario and Wayne counties,
ElderONE is an innovative alternative to long-term nursing home
placement. As the area’s only PACE program, ElderONE represents the
future of senior health care. This long-term care program provides
the entire continuum of medical care and support services to
seniors 55 and older with chronic care needs, allowing them to
maintain their independence in the comfort of their home without
compromising their health or safety. RESPONSIBILITIES - Patient
Care. Conduct initial assessments; establish plan of care and
provide prescribed treatments and demonstrated nursing abilities;
coordinate the delivery of 24-hour care; - Communication. Counsel
and guide individuals and families towards self-help in recognition
and solution of physical, emotional and environmental problems -
Evaluation & Documentation. Conduct reassessment of each patient on
a regular basis; maintain complete and accurate clinical
documentation; supervise and serve as a resource to all home care
staff Required Qualifications: • Registered Nurse license in New
York State. • Valid NYS Driver’s License Preferred Qualifications:
• Diploma or Associate’s Degree in Nursing required; Bachelor’s
Degree in Nursing preferred. • 1 year of nursing experienced
preferred. • Prior home health, clinical and direct patient care

Vacation & Holidays EDUCATION: AS: Nursing (Required) LICENSES /
CERTIFICATIONS: BLS - Basic Life Support - American Heart
Association (AHA), RN - Registered Nurse - New York State Education
Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work -
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25
pounds of force frequently, and/or greater than negligible up to 10
pounds of force constantly to move objects; Requires frequent
walking, standing or squatting. For disease specific care programs
refer to the program specific requirements of the department for
further specifications on experience and educational expectations,
including continuing education requirements. Any physical

RANGE: $73,528.00 - $98,072.00 CITY: Rochester POSTAL CODE: 14617
The listed base pay range is a good faith representation of current
potential base pay for a successful full time applicant. It may be
modified in the future and eligible for additional pay components.
Pay is determined by factors including experience, relevant
qualifications, specialty, internal equity, location, and
contracts. Rochester Regional Health is an Equal
